Physiotherapist or orthopaedic doctor — who first?

Most Indians assume every ache needs an orthopaedic doctor first. Often that's an extra appointment, an extra fee, and sometimes an unnecessary scan — for a problem a physiotherapist treats directly. Here's an honest guide to choosing, including when a physio is not the right first stop.

See a physiotherapist first
  • Back or neck pain without a specific injury or accident
  • Joint stiffness, posture problems, desk-work aches
  • Recovering after a surgery your doctor has already cleared for rehab
  • Sports strains and overuse pains (runner's knee, shoulder impingement)
  • Balance problems, fear of falling, general deconditioning
  • Chronic conditions where movement is the treatment — arthritis, spondylosis
See a doctor first
  • A significant injury — a fall, accident, or a joint that looks deformed
  • Inability to bear weight, or a suspected fracture
  • Severe unremitting night pain, fever with joint pain, or unexplained weight loss
  • Numbness around the groin, loss of bladder/bowel control (emergency)
  • Progressive weakness in a limb
  • When imaging (X-ray/MRI) or surgical opinion is clearly needed

Do I need a referral to see a physiotherapist in India?

In most cases, no — physiotherapy in India can be accessed directly for musculoskeletal aches and mobility problems. A good physiotherapist screens you at the first assessment and will refer you to a doctor if your problem needs medical investigation first. That screening is part of their training.

The two professions work best together: orthopaedic doctors diagnose and operate; physiotherapists restore movement and strength. After most orthopaedic surgeries, it's the quality of physiotherapy that decides the outcome.
